Denominator	Patients aged 18-75 seen in 12 months with a diagnosis of diabetes  who were documented as tobacco users in most recent tobacco screening.
Numerator	Received cessation counseling or cessation medication within 24 months. (MSSP 2019)
Exclusion/Exception	• Patients with hospice care in 12 months. • Documentation of medical reason(s) for not screening for tobacco use OR for not providing tobacco cessation intervention for patients identified as tobacco users (eg, limited life expectancy, other medical reason).
