Denominator	Patients 18 years of age and older who were documented as tobacco users and who were diagnosed with acute myocardial infarction (AMI), coronary artery bypass graft (CABG) or percutaneous coronary interventions (PCI) in 24 to 12 months prior, OR who had an active diagnosis of ischemic vascular disease (IVD) in 12 months. Numerator	Received cessation counseling or cessation medication within 24 months. (MSSP 2019)
Exclusion/Exception	• Documentation of medical reason(s) for not screening for tobacco use OR for not providing tobacco cessation intervention for patients identified as tobacco users (eg, limited life expectancy, other medical reason). • Patients with hospice care in 12 months.
